Abstract

This book chapter presents a case report and discussion on the use of the anterolateral thigh (ALT) flap for reconstructing a post-traumatic scrotal defect with exposed testicles following a road traffic accident. The case presentation describes the preoperative condition of the patient and the surgical procedure performed, which involved debridement and reconstruction using an islanded ALT flap. The flap was raised from the anterolateral thigh region, islanded, and transferred to cover the scrotal defect, while the donor site was grafted with split-thickness skin grafts.

The postoperative period remained uneventful, and satisfactory results were observed during follow-up examinations. The discussion section introduces the ALT flap technique, highlighting its versatility and reliability in reconstructive surgery. The surgical techniques involved in the procedure are outlined, including patient evaluation and planning, flap design and marking, flap elevation, flap transposition and inset, and donor site closure.

Advantages of the ALT flap technique include the availability of abundant tissue for reconstructing moderate to large defects, versatility in customizing the flap to match specific requirements, reliable vascular supply through the perforating branches of the descending branch of the lateral circumflex femoral artery, reduced donor site morbidity, and minimal impact on ambulation. The limitations of the technique, such as the need for detailed preoperative vascular imaging and the requirement for surgical expertise in microsurgical techniques, are also discussed.

The abstract emphasizes the practical application and benefits of the ALT flap technique in reconstructive surgery. By understanding the surgical techniques and considering the advantages and limitations of the ALT flap, healthcare professionals can effectively address complex soft tissue defects in various anatomical regions, ultimately improving patient outcomes.
