This book chapter presents a case report and discussion on Cronin's procedure, also known as the wedge resection technique, for the correction of syndactyly. The case presentation describes a child born with fused middle, ring, and little fingers, with the third web fusion extending to the tip of the distal phalanges and the fourth web fusion extending to the distal interphalangeal joint. The diagnosis is congenital simple complete third web and incomplete fourth web syndactyly of the left hand.
The operative procedure involves the release and reconstruction of the syndactyly using Cronin's technique and Buck-Gramcko flaps for nail fold reconstruction. The surgeon marks the areas for release and reconstruction, performs the wedge resection, and mobilizes the flaps to achieve third web and nail fold reconstruction. The postoperative period remains uneventful, and satisfactory results are observed during follow-up examinations.
The discussion section introduces Cronin's procedure, highlighting its purpose in correcting syndactyly by separating fused digits through the removal of a wedge-shaped portion of tissue and bone. The surgical techniques involved in the procedure are described, including patient evaluation and planning, anaesthesia, incision and exposure, wedge resection, reconstruction, wound closure, and postoperative care.
Advantages of Cronin's procedure include improved finger function, aesthetic improvement, and early intervention to optimize hand development. The limitations of the procedure, such as the need for an individualized approach based on the severity and type of syndactyly, and potential risks and complications, are also discussed.
The abstract emphasizes the practical application of Cronin's procedure in correcting syndactyly and the benefits it provides in terms of finger function and aesthetics. By understanding the surgical techniques and considering the advantages and limitations of the procedure, healthcare professionals can effectively address syndactyly and improve hand function and appearance in affected individuals.
