This book chapter presents a case report and discussion on Bardach's Two Flap Palatoplasty, a surgical technique used to repair cleft palate. The case presentation describes a child with a complete left side cleft lip and palate, where the cleft palate remained after a previous cheiloplasty for the left cleft lip. The diagnosis is a follow-up case of left complete cleft lip and palate, classified as Modified Kernhan's 10-19 and Veau group III.
The operative procedure involves the use of Bardach's Two Flap Palatoplasty technique to repair the cleft palate. The surgeon performs a two-layered closure for the hard palate, including closure of the intervening muscle layer along with the nasal mucosal and oral mucoperiosteal layers. The postoperative period remains uneventful, and satisfactory results are observed during follow-up examinations.
The discussion section provides an introduction to Bardach's Two Flap Palatoplasty, emphasizing its purpose in repairing cleft palate by achieving a tension-free closure using two flaps. The surgical technique is described, including patient evaluation and planning, anesthesia, incision and exposure, flap design and elevation, closure and suturing, and postoperative care.
Advantages of Bardach's Two Flap Palatoplasty include improved speech development, enhanced feeding and swallowing, aesthetic improvement, and reduced risk of complications. The limitations of the procedure, such as the need for an individualized approach and the requirement for multidisciplinary care, are also discussed. Potential complications and the importance of preoperative evaluation, surgical technique, and postoperative care are highlighted.
The abstract highlights the practical application of Bardach's Two Flap Palatoplasty in repairing cleft palate and the benefits it provides in terms of speech development, feeding and swallowing, and aesthetic improvement. By understanding the surgical technique and considering the advantages and limitations of the procedure, healthcare professionals can effectively address cleft palate and contribute to the overall well-being and development of affected individuals.
