Abstract

This book chapter focuses on Mulliken's Cheiloplasty, also known as the modified Millard repair, a surgical technique used for repairing cleft lip. A case presentation is provided, describing a child with bilateral incomplete cleft lip and cleft secondary palate. The diagnosis is bilateral incomplete cleft lip and cleft secondary palate, classified as Modified Kernhan's 2,3,6-9, 10, 12,13,16-19.

The operative procedure involves marking for Mulliken's Cheiloplasty, followed by flap preparation and the freeing of abnormal orbicularis oris muscle attachments. Muscle, skin, and mucosal approximation are performed to close the cleft lip. The postoperative period remains uneventful, and satisfactory results are observed during follow-up examinations.

The discussion section introduces Mulliken's Cheiloplasty, highlighting its purpose in repairing cleft lip and the modifications made to enhance aesthetic and functional outcomes. The surgical techniques involved in the procedure are described, including patient evaluation and planning, anesthesia, incision and exposure, tissue mobilization and alignment, tissue closure and suturing, and postoperative care.

Advantages of Mulliken's Cheiloplasty include improved lip aesthetics, functional improvement, an individualized approach, and minimized scarring. The limitations of the technique, such as the need for multidisciplinary care and potential complications, are also discussed. The chapter emphasizes the practical application of Mulliken's Cheiloplasty in repairing bilateral incomplete cleft lip and cleft secondary palate, highlighting the importance of comprehensive care, surgical expertise, and postoperative management.
